As a freelancer how does hourly rate actually work?

What I mean is how is it calculated, do you log your the amount of time you are working on the project and expect the client to trust that you spent that amount of time on the project? Is it estimated automatically to a 40 hour work week? Is it just what you guess how long you will spend on the project? I'm just wondering how exactly does it work.
